Journey Community Connections Pty Ltd, bought together by the unique paths of our own children. Dedicated to fostering a supportive community of support and empowerment for individuals with disabilities. 

Our team of Support Workers are able to provide ongoing Support to Self-Managed and Plan Managed Participants, between Grafton NSW and Coffs Harbour NSW, Our Team is Effective with Training and Upskilling, and each of our amazing Support Team Members, have Lived and Work experience throughout.

 Journey Community Connections celebrates Neurodiversity, assisting to build bridges of connections and champion inclusivity. Through our company, each participant brings a unique thread to our experience creating rich narratives of challenges, we aim to provide support, create a sense of belonging for families navigating the unique challenges and joys knowing that together we are not alone.
